Time-Restricted Eating

Time-restricted eating (TRE)—also called intermittent fasting or 16:8 eating—shows promise for metabolic health, but effect sizes are modest and the evidence base has significant limitations. It is not clearly superior to standard caloric restriction for most outcomes when calories are matched.

What the evidence suggests

  • TRE can reduce caloric intake in many people, likely because it shortens the eating window rather than through any distinct metabolic mechanism
  • Short-term studies (8–12 weeks) show modest improvements in insulin sensitivity, blood glucose and some cardiometabolic markers in people with overweight or metabolic risk factors
  • Circadian-aligned TRE (eating earlier in the day) may have additional benefits beyond simple caloric restriction in some studies
  • Adherence is variable but some individuals find time-based restriction easier to maintain than caloric counting

What remains uncertain

  • Whether benefits are primarily from caloric restriction or from the timing pattern itself remains contested
  • Long-term outcomes (beyond 12 months) are largely unstudied in randomised trials
  • Effects in people of normal weight or without metabolic risk factors are less established
  • Impact on muscle mass — particularly in people doing resistance training — requires more research
  • Individual responses vary considerably; what works for one person may not suit another

⚠ Safety considerations

TRE is not appropriate for pregnant or breastfeeding women, children, people with a history of disordered eating, or those with diabetes on insulin or certain medications without medical supervision. Discuss significant dietary changes with a healthcare professional.
